PTAC Meetings

The Committee meets publicly on approximately a quarterly basis. As required by the Federal Advisory Committee Act (FACA), notifications, agendas, and materials relating to the meeting are provided to the public in a timely manner. Please see individual meetings below.

PTAC’s September 2024 meeting focused on:

  • Stakeholder perspectives on developing a pathway toward having all Medicare beneficiaries with Parts A and B in care relationships with accountability for quality, outcomes, and TCOC;
  • Envisioning future PB-TCOC models, the needs of different kinds of participating PB-TCOC organizations, and necessary components for success;
  • Organizational structure, payment, and financial incentives for supporting accountable care relationships;
  • Developing a balanced portfolio of performance measures for PB-TCOC models; and
  • Addressing challenges regarding data, attribution, benchmarking, and risk adjustment.

PTAC’s public meeting focused on addressing the needs of patients with complex chronic conditions or serious illnesses in population-based total cost of care (PB-TCOC) models. Topics regarding this patient population included issues that have been raised in several previously submitted physician-focused payment model proposals, such as:

  • Care delivery needs of patients who account for the top 5% of Medicare spending;
  • Strategies for improving patient-centered care, care delivery, & health outcomes;
  • Optimizing the use of post-acute care, palliative care, & end-of life care;
  • Approaches for measuring performance and quality of care; and
  • Incentivizing improved outcomes and delivery system transformation.

PTAC’s March public meeting focused on developing and implementing performance measures for PB-TCOC models and issues that have been raised in several previously submitted physician-focused payment model proposals. Topics included:

  • Defining performance measurement objectives;
  • Selecting and balancing between the number and types of performance measures;
  • Best practices for linking performance measures with payment and financial incentives;
  • Addressing challenges related to implementing performance measures; and
  • Incorporating health equity and the patient experience into performance measures.

PTAC's 2023 series of theme-based discussions focused on key issues related to operationalizing Population-Based Total Cost of Care (PB-TCOC) models. PTAC’s September 2023 public meeting focused on encouraging rural participation in PB-TCOC models. Topics included challenges facing patients and providers in rural communities, approaches for incorporating rural providers in PB-TCOC model design, provider perspectives on payment issues related to rural providers in PB-TCOC models, incentives for increasing rural providers’ participation in PB-TCOC models, and successful interventions and models for encouraging value-based transformation in rural areas. This meeting also addressed important issues that have been raised in several physician-focused payment model (PFPM) proposals that have previously been submitted to PTAC.

This all built on the Committee’s ongoing series of theme-based discussions related to developing and implementing PB-TCOC models – including recent discussions on improving care delivery, specialty integration and care transition management.

Building on the Committee’s 2022 series of theme-based discussions on Population-Based Total Cost of Care (PB-TCOC) models and the March 2023 discussion on improving care delivery and specialty integration in population-based models, PTAC’s June 2023 meeting focused on improving care transition management within PB-TCOC models. Topics included: innovative approaches for improving management of care transitions across settings of care, designing financial incentives for improving care transition management, addressing care transitions in APM design, and measuring care transition quality.

PTAC’s March 2023 public meeting focused on specialist integration within Population-Based Total Cost of Care (PB-TCOC) models and physician-focused payment models. Topics discussed included improving specialty integration in advanced primary care models and Accountable Care Organizations; approaches for structuring coordination between primary care providers and specialists; options for defining and embedding specialty episodes within population-based models; structuring financial incentives; identifying appropriate performance measures; and addressing challenges affecting safety net providers and rural providers.

The September meeting was part of a series of theme-based meetings PTAC held to inform the Committee on topics important to population-based total cost of care (PB-TCOC) models and physician-focused payment models (PFPMs) throughout 2022. The meeting focused on payment considerations and financial incentives related to PB-TCOC models. The first meeting in the series centered on key definitions, issues, and opportunities related to these models, and the second meeting addressed care delivery model design.
