LTSS Federal Update

Goal 1: Research

  • Agency for Healthcare Research & Quality (AHRQ):
    • University of MN is evaluating the use of remote sensory technology to help manage persons with ADRD and determine its impact on the ability of the family caregiver to manage the person with ADRD, impact on caregiver distress, reduction in health declines and health care utilization. It is beginning its 3rd year and will be finished in 2019. Recruitment of ADRD caregivers is continuing. More than 60 have been recruited, systems installed and caregivers trained. The recruitment goal is 100.
    • University of WI is disseminating and evaluating “Elder Tree.” This is an information and communication HIT- based tool designed to help reduce loneliness and isolation and to enhance access to services and supports for older adults with disabilities and their caregivers. Components include: social networking through discussion groups with other users, check in service to help identify problems and to help provide access to appropriate services, healthy reminders combined with help linking to local resources, “ask the health coach” to provide access to specialists in areas such as falls prevention, diabetes care, nutrition, and caregiving, medication resources, a calendar that tracks appointments and provides reminders for medications, exercise, and tracks changes in behavior, and a route planning tool to help with trip planning. Approximately 400 people have been recruited from 3 counties in Wisconsin to use this tool and the participants have been surveyed. Analysis is underway and the grant is expected to be completed by June, 2016.
  • Assistant Secretary for Planning and Evaluation: Study is examining dementia care models across settings using a framework to understand what implementation challenges providers face. Project involves an environmental scan and a series of five case studies. Results will inform future research related to developing quality measures and standards. Project will be complete September 2016.

Goals 2: Services Expansion & Quality

  • Administration for Community Living: ACL issued one of two FY2016 Funding Opportunity Announcements (FOA) for Alzheimer’s and Dementia Programs.
    • Alzheimer’s Disease Supportive Services Program (ADSSP) grantees 1) enhance the ability of state systems and programs to embed dementia-capability in their service networks; and 2) deliver dementia-capable supportive services using evidence-based and/or evidence-informed interventions to support persons with dementia and their caregivers. Applications due May 23. Mandatory letters of intent due April 5.
    • Alzheimer’s Disease Initiative -- Specialized Supportive Services (ADI-SSS) will be released at the end of April. ADI-SSS is designed to fill specifically identified service gaps for persons with dementia and their caregiver.
  • Indian Health Service: REACH into Indian Country. April 15, 2016 -- 80 staff received training and 42 now have REACH certification . Program is available in 29 communities, with 12 caregivers enrolled.
  • Department of Veterans Affairs: VA and ACL are doing a webinar on 5/5/16 to discuss the VA’s report on completed non-institutional long-term care pilot programs with dementia components. Focus will be on one new model, Mobile Adult Day Health Care.  It involves community partnerships to provide meeting space for a mobile team to provide adult day health care to individuals with dementia. This increases access to day care.

Goal 3: Quality

  • ACL Office of LTC Ombudsman Program:
    • Emergency Preparedness and Response -  The National Ombudsman Resource Center (NORC) provided in-person training of State LTC Ombudsman programs in April, 2016 with an emphasis on building coalitions to address emergency preparedness and response including lessons learned from 3 pilot states (CO, AK, HI)
    • ACL staff continues to work with states regarding the implementation of the LTC Ombudsman rule, which becomes effective on July 1, 2016. ACL/AoA’s Regional Support Centers are incorporating LTCO Rule implementation by states into their 2016 annual reviews.
    • ACL and NORC provided in-person training (April 2016) and technical assistance on person-centered complaint processes, and  health care decision-making including examples of supporting individuals living with dementia, building upon past NAPA training activities.
