The purpose of this evaluation was to assess the impact of the "Home Health Aide (HHA) Partnering Collaborative" a quality improvement initiative implemented at the Visiting Nurse Service of New York in three main areas: (1) patient outcomes; (2) patient service use; and (3) aide job perceptions and retention. The Collaborative was designed to improve the quality of work life and retention of home care paraprofessionals, as well as increase clinicians' and aides' support for patients' self-management and improvement in key activities of daily living.
