Support and Services at Home (SASH) Evaluation: First Annual Report September 2014 RTI International Abstract The Support and Services at Home (SASH) program in Vermont is a subcomponent of a larger Medicare demonstration--the Multi-payer Advanced Primary Care Practice demonstration. SASH is a model of care coordination centered on affordable housing sites for older adults. This is an interim report with preliminary findings. These findings indicate that SASH is a promising intervention for reducing the growth of Medicare expenditures.
