DESCRIPTION: The Hispanic Health and Nutrition Examination Survey (HHANES) is a survey of three subgroups of the Hispanic population in selected areas, comprising about 76% of the 1980 Hispanic-origin population of the United States. A stratified multistage probability sample was used. The survey was designed to produce estimates of the health and nutritional status of noninstitutionalized civilians, ages 6 months through 74 years, in families containing at least one member who belonged to the targeted Hispanic subgroup.

TITLE: National Health Interview Survey

DESCRIPTION: The National Health Interview Survey (NHIS) is a household interview survey conducted continuously throughout the year. Each week's sample is a national probability sample of the civilian, noninstitutionalized population of the 50 States and the District of Columbia. Face-to-face interviews are conducted in sampled households. The purpose of the survey is to provide general health statistics on the Nation's population. A major strength of this survey lies in its ability to display many health characteristics through many demographic and socioeconomic factors.

NHIS has been conducted continuously since 1957, the content of the survey has been updated about every 10 - 15 years. In 1996 a substantially revised NHIS content began field testing. This questionnaire began in 1997 and improves the ability of the NHIS to provide important health information.

FILES CONTAIN DATA ON: limitations of activity, injuries, health insurance, access to health care, health care utilization, conditions, behaviors (including tobacco, physical activity, and alcohol), immunizations, and AIDS.

RACE/ETHNICITY: Racial Categories on the public use file are: White only, Black/African American only, American Indian/Alaska Native only; Asian only, Other race only, and Multiple Race. Data is gathered on additional race categories (Native Hawaiians, Guamanian, Samoan, Other Pacific Islanders, Asian Indian, Chinese, Filipino, Japanese, Korean, Vietnamese) which are not included on the public use file for confidentiality reasons.

Ethnic Categories are: Hispanic (Multiple Hispanic, Puerto Rican, Mexican, Mexican-American, Cuban/Cuban American, Dominican (Republic), Central or South American, Other Latin American, Other Spanish, Hispanic/Latino/Spanish type unknown); and Not Hispanic/Spanish origin.

DATA LIMITATIONS: Large sampling errors of estimates for small populations.

STATUS: Starting in July 1957, this continuous data collection is active.

TITLE: National Health Examination Survey I (1960-62); National Health Examination Survey II (1963-65); National Health Examination Survey III (1966-70)

DESCRIPTION: The First National Health Examination Survey (NHES I), 1960-62, focused mainly on selected chronic disease of adults ages 18-79. The sample for NHES I, II, and III was selected to represent civilian noninstitutionalized adults residing in the coterminous United States. The three survey cycles used a stratified multistage probability sample design. The second cycle of the National Health and Nutrition Examination Survey (NHES II) focused primarily on the growth and development of children, ages 6-11 years.

The third cycle of the National Health Examination Survey (NHES III) focused on the growth and development of children ages 12-17 years. Some of these children had been examined in NHES II, allowing for analysis of longitudinal data.

TITLE: The First National Health and Nutrition Examination Survey, 1971-74 (NHANES I) and the National Health and Nutrition Augmentation Survey of Adults 25-74 Years of Age, 1974- 75 (NHANES I Augmentation)

DESCRIPTION: The First National Health and Nutrition Examination Survey (NHANES I), 1971-74 and 1974-75, is a cross-sectional study of the U.S. population developed for the collection and dissemination of health and nutrition information. A stratified multistage probability sample design was used. Obtained by interview and by direct physical examination, the data permit the estimation of the prevalence of specific diseases and the distribution of a wide variety of health related measures for analyses.

Another important purpose of NHANES I was to measure and monitor indicators of nutritional status of the population. This was accomplished through the collection of dietary intake data, a general physical examination, body measurements, biochemical tests, and clinical assessments for evidence of nutritional deficiency. Although the survey targeted civilian, noninstitutionalized persons ages 1-74, the sample was more heavily weighted on "high-risk" groups such as: low-income, older age, preschool children, and women of childbearing age.

TITLE: The Second National Health and Nutrition Examination Survey, 1976-80

ACRONYM: NHANES II

DESCRIPTION: The Second National Health and Nutrition Examination Survey (NHANES II), 1976-80, continued the responsibility begun in NHANES I to monitor the U.S. population's health and nutritional status. Repeated procedures and content items from NHANES I permitted comparison and measurements of changes over time. The sample for NHANES II was civilian noninstitutionalized persons ages 6 months through 74 years. A stratified multistage probability sample design was used.

Nutritional assessment in NHANES II used essentially the same format as NHANES I, with some modifications. Information was collected in NHANES II on participation in food stamp programs and home delivery meals, allowing comparison with those of similar socioeconomic status who do not participate. In addition to the specific illness or "target conditions" data collected in NHANES I, several new conditions were "targeted" in NHANES II. These included diabetes, kidney pathology, liver function, allergy, and blood tests for environmental pollution.

TITLE: The Third National Health and Nutrition Examination Survey, 1988-94

ACRONYM: NHANES III

DESCRIPTION: The Third National Health and Nutrition Examination Survey (NHANES III), 1988-94, is the seventh in a series of studies developed to collect health and nutrition information on the U.S. civilian noninstitutionalized population. A stratified multistage probability design was used. Participants were interviewed and given a direct physical examination. The need for descriptive data on the health status of selected population groups (children ages 2 months to 5 years and persons over 60 years) led to these groups being sampled in large proportions. Previous surveys showed that some minority groups can have a very different health status and characteristics. Over sampling of black Americans and Mexican-Americans (30% each of the total sample population) will allow investigation of the risk factors that may explain racial and ethnic differences.

TITLE: NHANES I Epidemiologic Follow up Study

DESCRIPTION: The NHANES I Epidemiologic Follow up Study (NHEFS) was designed to investigate the relationships between clinical, nutritional, and behavioral factors assessed in the first National Health and Nutrition Examination Survey (NHANES) and subsequent morbidity, mortality, and hospital utilization, as well as changes in risk factors, functional limitation, and institutionalization. The NHEFS cohort includes all persons between 25 and 74 years of age who completed a medical examination at NHANES I (1971-74; N=14,407). It comprises a series of follow up studies, four of which have been conducted to date. The first wave of data collection was conducted for all members of the cohort from 1982 to 1984. It included tracing the cohort; conducting personal interviews with subjects or their proxies; measuring pulse rate, weight, and blood pressure of surviving participants; collecting hospital and nursing home records of overnight stays; and collecting death certificates of decedents.

Continued follow-ups of the cohort were conducted in 1986, 1987, and 1992 using the same design and data collection procedures developed in the 1982-84 NHEFS, with the exception that a 30-minute computer-assisted telephone interview was administered rather than a personal interview and no physical measurements were taken. The 1986 cohort includes subjects who were 55-74 years of age at their baseline examination and not known to be deceased at the 1982-84 interview (N=3,980). The 1987 NHEFS was conducted for the entire nondeceased cohort (N=11,750). The fourth wave of data collection, the 1992 survey includes the entire nondeceased cohort (N=11,195). Public use files are available for all four followup studies. NHEFS public use data tapes contain information on vital and tracing status, subject and proxy interviews, health care facility stays in hospitals and nursing homes, and mortality data from death certificates.

TITLE: National Ambulatory Medical Care Survey

ACRONYM: NAMCS

DESCRIPTION: The National Ambulatory Medical Care Survey collects data on ambulatory care visits made to physician offices in the United States. Findings are based on a national sample of visits to nonfederally employed, office-based physicians, excluding those who specialize in anesthesiology, pathology, and radiology. The survey uses a three-stage probability design with samples of geographically defined areas, physicians stratified by specialty within these areas, and patient visits to physicians. The survey was conducted annually between 1973-1981, again in 1985, and resumed an annual schedule in 1989.

Specially trained interviewers visit the physicians prior to their participation in the survey in order to provide them with survey materials and instruct them on how to complete the forms. Data collection from the physician, rather than from the patient, provides an analytic base that expands information on ambulatory care collected through other NCHS surveys. Each physician is randomly assigned to a 1-week reporting period. During this period, data for a systematic random sample of visits are recorded by the physician or office staff on an encounter form provided for that purpose.

Files contain data on: patient demographic characteristics (age, sex, race, ethnicity), expected source of payment, patient's reason for visit, injury-related information, physician's diagnosis, diagnostic/screening services, therapeutic and preventive services including medication, providers seen, visit disposition, time spent with physician, and physician practice characteristics.

RACE/ETHNICITY: Racial Categories are: American Indian/Alaska Native; Asian; Black/African American; Native Hawaiian and Other Pacific Islander; and White.

Ethnic Categories are: Hispanic or Latino; and Not Hispanic or Latino.

DATA LIMITATIONS: The information on race and ethnicity is based on the physician's knowledge of the patient or by observation. The physician is not directed or expected to ask the patient for this information. About 18 percent of the sample records lack data on race, and about 23 percent lack data on ethnicity.

STATUS: Conducted annually from 1973-1981, again in 1985, and resumed an annual schedule in 1989.

For survey years 1973-1979, there are single files where each record contains information on a sampled patient visit. With the addition to the survey of medication data in 1980, two data files were produced for survey years 1980, 1981, 1985, 1989, 1990, and 1991 -- one for patient visit data and a second for drug mention data where each record represents a single drug mention along with its associated visit data. The second file is limited to those visits with mention of medication therapy. For the 1991 data, it is possible to link information on the drug file with information on the patient visit file. Beginning with the 1992 survey year, only one data file is produced annually that contains both patient visit and drug information.

TITLE: National Hospital Ambulatory Medical Care Survey

DESCRIPTION: The National Hospital Ambulatory Medical Care Survey (NHAMCS) collects data on the utilization and provision of ambulatory care services in hospital emergency and outpatient departments. Findings are based on a national sample of visits to the emergency and outpatient departments of noninstitutional general and short-stay hospitals, excluding Federal, military, and Veterans Administration hospitals, within the United States. The survey uses a four-stage probability design with samples of geographically defined areas, hospitals within these areas, clinics within hospitals, and patient visits within clinics. Annual data collection began in 1992.

Specially trained interviewers visit the hospitals prior to their participation in the survey to explain survey procedures, verify eligibility, develop a sampling plan, and train hospital staff in data collection procedures. The survey instrument is the Patient Record form, which is provided in two versions, one for use in outpatient departments and another for use in emergency departments. Hospital staff are instructed to complete Patient Record forms for a systematic random sample of patient visits during a randomly assigned 4-week reporting period.

Files contain data on: patient demographic characteristics (age, sex, race, ethnicity), expected source of payment, patient's reason for visit, injury-related information, physician's diagnosis, diagnostic/screening services, procedures, medication therapy, providers seen, visit disposition, and hospital characteristics such as type of ownership. The emergency department file also contains data on mode of arrival, immediacy with which patient should be seen, and presenting level of pain.

RACE/ETHNICITY: Racial Categories are: American Indian/Alaska Native; Asian; Black/African American; Native Hawaiian and Other Pacific Islander; and White.

Ethnic Categories are: Hispanic or Latino; and Not Hispanic or Latino.

DATA LIMITATIONS: The information on race and ethnicity is based on the hospital staff's knowledge of the patient or by observation. The hospital staff is not directed or expected to ask the patient for this information. About 12% of the encounter forms do not have race reported and about 18% do not have ethnicity.

DESCRIPTION: The National Survey of Ambulatory Surgery provides annual information for 1994 - 1996 on the use of hospitals and surgicenters for ambulatory surgery in the United States. Data are collected on diagnoses, surgical procedures, and patient characteristics from a national sample of 418 non-Federal, short-stay hospitals and 333 free-standing surgicenters or approximately 10% of the universe of facilities performing ambulatory surgery. The information is abstracted from a sample of medical records from each sample facility for a total sample of about 125,000 records each year. The record unit in the database is an ambulatory surgery visit.

DESCRIPTION: The National Health Provider Inventory provides data on services, location, staff, capacity, and other characteristics of selected health care providers in the United States. Information is collected via mail questionnaire with telephone follow up to all providers (100% census) of selected types. In 1991 (the last time the inventory was conducted), the inventory included nursing and personal care homes, licensed board and care homes, home health agencies, and hospices. The source of the information is the financial and administrative records of the provider. The individual provider is the record unit in the inventory database.

TITLE: National Home and Hospice Care Survey

DESCRIPTION: The National Home and Hospice Care Survey is a continuing series of surveys of home and hospice care agencies in the United States. The survey was conducted in 1992, 1993, 1994, 1996, 1998, and 2000. Information was collected about agencies that provide home and hospice care and about their current patients and discharges.

Home health agencies and hospices are usually defined in terms of the types of care they provide. Home health care is provided to individuals and families in their place of residence for the purpose of promoting, maintaining, or restoring health or for maximizing the level of independence while minimizing the effects of disability and illness including terminal illness. Hospice care is defined as a program of palliative and supportive care services providing physical, psychological, social, and spiritual care for dying persons, their families, and other loved ones. Hospice services are available in both home and inpatient settings.

The sampling frame for the 2000 National Home and Hospice Care Survey (NHHCS) consisted of 15,451 agencies classified as agencies providing home health and hospice care and the frame includes all agencies that are licensed or certified (Medicare and Medicaid). The universe of home health agencies and hospices was obtained from various national organizations and other sources.

The survey uses a two stage probability sampling design. In the first stage, a stratefied sample of facilities was taken. Facilities were stratefied by type cility (home health agencies, hospices, and mixed agencies), Metropolitan Statistical Area, region, and certification status. In the second stage, lists of current residents and discharges were constructed for each agency so that six current residents and six discharges could be selected using a systematic probability sample. Included were discharges that occurred because of the death of the patient. A total of 1,425 facilities, current residents, and discharges were included in the 2000 survey.

The National Home and Hospice Care Survey provides information on home health and hospice agencies, their current patients, and discharges. Files contain data on: referral on length of service, diagnoses, number of visits, patient charges, health status, reason for discharge, and types of services provided.

RACE/ETHNICITY: Racial categories used for the 2000 survey were: American Indian/Alaskan Native; Asian; Native Hawaiian or Other Pacific Islander; Black; White; and Other; and Don't Know. The data collection form permits identification of more than one race. Hispanic/Latino origin was collected as Yes, No, Don't know.

DATA LIMITATIONS: Race is unknown for about 20% of the sampled records, and ethnicity is unknown for about 30%--40% of the records.

STATUS: Started in September 1992, this periodic data collection is active. The survey has been conducted in 1992, 1993,1994, 1996, 1998, and 2000.

DESCRIPTION: The National Hospital Discharge Survey provides information annually on the inpatient use of hospitals in the United States. Data are collected on diagnoses, surgical and nonsurgical procedures, and patient characteristics from a national sample of approximately 500 non-Federal, short-stay hospitals or approximately 8% of the universe. The information is abstracted from a sample of medical records from each sample hospital for a total sample of about 270,000 records each year. The record unit in the database is a hospital discharge. Medical information is coded according to the International Classification of Diseases, 9th Revision, Clinical Modification.

TITLE: National Nursing Home Survey

DESCRIPTION: The National Nursing Home Survey is a continuing series of national sample surveys of nursing homes, their residents, and their staff. The survey was conducted in 1973-74, 1977, 1985, 1995, 1997, and 1999. Although each of these surveys emphasized different topics, they all provide some common basic information about nursing homes, their residents, and their staff. The NNHS is currently being redesigned with pilot testing scheduled to begin in early 2003 and data collection for the redesigned survey to begin in late 2003.

The most recent survey (conducted in 1999), used a stratified two-stage probability sample design. In the first stage, facilities were selected from a sample frame consisting of nursing homes with 3 or more beds, those certified by Medicare or Medicaid, and those with a state license to operate as a nursing home. Facilities were stratefied by bed size and certification status and nursing homes were selected using systematic probability sample. In the second stage, lists of current residents and discharges were constructed for each facility, so that six current residents and six discharges could be selected using a systematic probability sample. Discharges that occurred because of death are included in the sample. A total of 1,423 facilities, 8,215 current residents and 6,913 discharges were included in the survey. Data were collected personal interviews with administrators and staff using facility records. No resident was interviewed directly.

The National Nursing Home Survey provides information on nursing homes from two perspectives--that of the provider of services and that of the recipient. Data about the facilities include characteristics such as size, ownership, Medicare/Medicaid certification, occupancy rate, number of days of care provided, and expenses. For recipients, data are obtained on demographic characteristics, health status, and services received.

RACE/ETHNICITY: Racial categories used for data collection in the 1999 survey are: American Indian/Alaska Native; Asian; Native Hawaiian or Other Pacific Islander; Black or African American; White; Other; and Don't Know. The data collection form permits identification of more than one race. Hispanic/Latino origin was collected as: Yes, No, Don't know.

STATUS: The survey has been conducted in 1973-74, 1977,1985, 1995, 1997, and 1999. The survey is currently under redesign to broaden the content. The NNHS is currently being redesigned with pilot testing scheduled to begin in early 2003 and data collection for the redesigned survey to begin in late 2003.

TITLE: 1988 National Maternal and Infant Health Survey

DESCRIPTION: The National Maternal and Infant Health Survey (NMIHS) provides data on maternal and infant health, including prenatal care, birth weight, fetal loss, and infant mortality. The 1988 survey was based on three national samples of vital records: 9,935 birth certificates, 5,332 infant death certificates, and 3,309 fetal death certificates. Black mothers were over sampled in the natality component; data are weighted for production of national estimates.

Information was obtained by mailed questionnaires from mothers on: barriers to prenatal care; source of payment; Women, Infants, and Children use patterns; smoking, drinking, and marijuana use; work patterns before and after delivery; infant feeding practices; infant health and medical care up to 6 months; and sociodemographic characteristics. Information was obtained similarly from hospitals on: maternal hospitalizations--prenatal and up to 6 months postpartum; maternal and infant diagnoses and procedures (in ICD-9); charges for care and Diagnosis-Related Groups; cesarean delivery and trial of labor; fetal monitoring; medical devices, apnea monitors, and respirators; infant resuscitation and neonatal intensive care; and infant hospitalizations up to 6 months. In addition, the following information was obtained from prenatal care providers: at each prenatal visit--weight, blood pressure, hematocrit, urine glucose, urine protein, hemoglobin, patient education, advice and referral, AIDS and sexually transmitted disease testing, sonograms and X-rays, prescribed medications and vitamins, amniocentesis and chorionic villus sampling, and charges for care. A 1991 follow up survey was conducted for those mothers who had a live birth and participated in the 1988 survey to obtain information on the health and development of the child. Information was obtained from the mother about her race/ethnicity and that of the baby's father.

DESCRIPTION: The 1991 Longitudinal Follow up creates a representative national longitudinal database for studying the respondents in the 1988 National Maternal and Infant Health Survey (NMIHS) by interviewing the women from the nationally representative NMIHS 2 years later. Child development, effects of low birth weight, use of Women, Infants, and Children foods, child nutrition, child care, child neglect, barriers to pediatric care, environmental hazard exposures, and injury reduction are some of the national health issues affecting children. The 1991 Longitudinal Follow up provides the comprehensive data needed by Federal, state, and private researchers to study factors related to early childhood morbidity and health.

TITLE: National Survey of Family Growth

ACRONYM: NSFG

DESCRIPTION: The National Survey of Family Growth (NSFG) provides information on childbearing, contraception, and closely related aspects of maternal and child health from a sample of women in the United States (including Alaska and Hawaii). Data are based on interviews with a probability sample of women ages 15-44 in each survey year or cycle for 1973, 1976, 1982, 1988 and 1995. In cycles 1 and 2 (1973 and 1976), the survey was limited to women who had ever been married or had their own children living with them. In cycles 3, 4, and 5 (1982,1988 and 1995), all women ages 15-44 were included. Data are collected through face-to-face interviews of women in their own households. Sample sizes were 9,797 in cycle 1, 8,611 in cycle 2, 7,969 in cycle 3, 8,450 in cycle 4, and 10,847 in cycle 5. All cycles included oversamples of black women. Cycle 5, conducted in 1995, included oversamples of black and Hispanic women. The next NSFG, cycle 6, is being conducted in 2002, and includes interviews with males as well as females. It includes oversamples of black, Hispanic, and teenaged males and females.

The survey is similar in design and purpose to the Growth of American Families studies conducted by the Scripps Foundation and the University of Michigan in 1955 and 1960 and to the National Fertility Surveys conducted by the Office of Population Research, Princeton University, in 1965 and 1970. These surveys provide comparable data on trends in some of the factors affecting the birth rate and reproductive health from 1955 through 1995.

Topics covered in the interview include: pregnancies and births; sexual activity; contraception; cohabitation; marriage and divorce; child care; adoption; fecundity; infertility and sterility; prenatal medical care; use of family planning services; birth expectations; ethnic and racial background; education; religion; employment; occupation; and income.

Information on contraception was obtained for each pregnancy interval, and more detailed contraceptive information was obtained for each month in the 3 years before the interview. A series of questions was also asked to establish whether each pregnancy was planned by the woman and her partner.

DESCRIPTION: Fetal death data are one component of the national vital statistics program and directly provide national, state, and local demographic and health data on fetal deaths based on the reports of fetal deaths occurring annually in the United States. Fetal death data are also used in combination with live birth and infant death records to provide information on perinatal mortality and pregnancy outcomes. Most areas require reporting all fetal deaths of gestations of 20 weeks or more; some require reporting of all periods of gestation. The detailed data files include unit record data for each fetal death reported. Fetal death data have been collected since 1918, have been published nearly as long, and have been available on electronic media since 1982. Race and ethnicity categories are the same as those for national mortality statistics. Beginning in 1992, the file separately identifies Asian Indian, Korean, Samoan, Vietnamese, and Guamanian for eight registration areas.

TITLE: National Vital Statistics System - Linked Birth-Infant Data Set

DESCRIPTION: The linked birth/infant death data set was created to facilitate more detailed research on infant mortality. In this data set, the information from the birth certificate is linked to the information from the death certificate for each of the entire population of infants who die in the United States each year. The purpose of the linkage is to utilize the many additional variables available from the birth certificate in infant mortality analysis. The linked data set contains two separate data files: a numerator file containing linked birth and death certificate information for all infant deaths in the United States in a particular year, and a denominator file containing information on all births in the United States in the same year. The linked data set is currently available for the 1983-91 and 1995-99. The 2000 linked file should be available by December 2002.

DESCRIPTION: The vital statistics general mortality data are a fundamental source of demographic, geographic, and cause-of-death information. This is one of the few sources of comparable health-related data for small geographic areas and a long time period in the United States. The data are also used to present the characteristics of those dying in the United States, to determine life expectancy, and to compare mortality trends with other countries. Preliminary and final annual data are available.

Files contain data on: Demographic information (age, race, Hispanic origin, sex, marital status, education), Geographic detail (state and county of residence and occurrence), Cause of death, and Other variables (month of death, place of death, etc.).

TITLE: National Mortality Followback Survey (1993)

DESCRIPTION: The 1993 National Mortality Followback Survey (NMFS) is the latest in a series of periodic surveys designed to supplement information routinely collected on the death certificate. This most recent NMFS is based on a sample of 22,977 persons ages 15 years and over who died in 1993. Supplemental information is obtained from interviews with the decedent's next-of-kin, identified as having provided information on the death certificate, and from medical examiner/coroner records if the death was referred to medical examiner/coroner for certification of death. The 1993 survey focuses on five broad subject areas: socioeconomic differentials in mortality, associations between risk factors and cause of death, access and utilization of health care services in the last year of life, disability, and reliability of certain items reported on the death certificate. Files contain data on: Functional/Health Status; Services Utilization; Services Expenditure and Financing; Socioeconomic; Age/Gender; Other Demographic /Sociocultural; Behavioral; and Other (medical examiner/coroner data, i.e., events surrounding death, autopsy, toxicology).

RACE/ETHNICITY: Racial Categories are: White; Black; Indian (American); Eskimo; Aleut; Chinese; Filipino; Hawaiian; Korean; Vietnamese; Japanese; Asian Indian; Samoan; Guamanian; Other Asian/Pacific Islander; and Other Race. ETHNICITY: Ethnic categories are: Puerto Rican; Cuban; Mexican/Mexicano; Mexican-American; Chicano; Other Latin American; and Other Spanish.

DATA LIMITATIONS: Although poststratified to national mortality totals, deaths in certain race categories (Eskimo, Aleut, specific Asian and Pacific Islanders) and ethnic categories (Cuban, Chicano, Other Latin American) are so few it will be necessary to combine into broader categories for analytical purposes. A record is provided for each death, with specific race detail.

STATUS: Started during January 1961 to December 1961, this intermittent data collection is no longer active.

HOW TO ACCESS DATA: Public use files, usable with restrictions

All personal identifiers are deleted, including state and substate geographic codes. All facility identities including medical examiner/coroner officers and personnel are also deleted. Special tapes may be made available to certain Federal agencies subject to written signed agreements outlining adherence to Section 308(d) of the Public Health Service Act (Title 42, USC, Section 242m) and the National Center for Health Statistics/Centers for Disease Control and Prevention staff manual on confidentiality nondisclosure statement. A public use data file containing information from the proxy respondents was released on CD-ROM in April 1998.

TITLE: State and Local Area Integrated Telephone Survey

DESCRIPTION: The State and Local Area Integrated Telephone Survey (SLAITS) is a telephone interview mechanism that employs the sampling frame of the National Immunization Survey. The purpose of SLAITS is to provide high quality health and welfare-related data on individuals and families at the State and local levels for policy and program evaluation. SLAITS is funded completely by survey sponsors and questionnaire modules are designed to meet the data requirements of sponsors. Survey sponsors include federal, State, and nonprofit agencies. One of the major advantages of the SLAITS mechanism is its flexibility with regard to sample design, questionnaire content, and timing of data collection. Efforts are made to use questions from existing national surveys for comparability. SLAITS employs standardized survey methodology and a consistent mode of administration in all geographic areas. Because SLAITS can be implemented rapidly and data are available within months of collection, it is particularly useful for emergent data needs.

RACE/ETHNICITY: Racial categories are: White, Black or African American, Asian, American Indian or Alaskan Native, Native Hawaiian or Other Pacific Islander (Multiple responses allowed). Ethnic categories are: Spanish, Hispanic, or Latino descent or national origin such as Mexican, Puerto Rican, or Cuban (Yes/No).

DATA LIMITATIONS: Multivariate analysis may be limited due to sample size at the subnational level.

STATUS: Started in January 1997, this is an intermittent data collection mechanism.

TITLE: The National Health and Nutrition Examination Survey, 1999+

ACRONYM:

DESCRIPTION: The National Health and Nutrition Examination Survey (NHANES), will become a continuous annual survey beginning in 1999. It will collect health and nutrition information on the U.S. civilian noninstitutionalized population. A stratified multistage probability design will be used. Participants will be interviewed and given a direct physical examination.

The need for descriptive data on the health status of selected population groups (adolescents ages 12-19 years and persons over 60 years) will result in these groups being sampled in large proportions. Previous surveys showed that some minority groups can have a very different health status and characteristics. Over sampling of black Americans and Mexican-Americans (30% each of the total sample population) will allow investigation of the risk factors that may explain racial and ethnic differences.

TITLE: National Asthma Survey

AGENCY/PROGRAM: National Center for Health Statistics, Centers for Disease Control and Prevention

DESCRIPTION: Sponsored by the National Center for Environmental Health, CDC, this SLAITS module will provide data on the prevalence of asthma in four states and nationally. Information about symptoms, treatment, impact, and environment will be collected on adults and children. Approximately 9,000 households will be screened resulting in 1,200 detailed interviews in Alabama, Illinois, California, Texas and nationally. A control sample of households without asthma will provide data on home environment.

RACE/ETHNICITY: Racial categories are White, Black or African American, Native American, Alaskan Native, Asian, Native Hawaiian, Pacific Islander, Other, and Multiple. Ethnicity is Hispanic or Latino (Yes/No).

DATA LIMITATIONS: Multivariate analysis may be limited at the state level. Racial detail beyond White, Black, Multi, Other is not released at the state level to protect the identity of survey respondents.

STATUS: The survey will be conducted January-December 2003. Funding permitting, additional states will be surveyed in subsequent years.

TITLE: National Survey of Children's Health

ACRONYM: NSCH

AGENCY/PROGRAM: National Center for Health Statistics, Centers for Disease Control and Prevention

DESCRIPTION: This SLAITS module, sponsored by the Maternal and Child Health Bureau, Health Resources and Services Administration, examines the physical and emotional health of children 0-17 years of age. Special emphasis is placed on factors that support the well-being of the child, including having a medical home, family interactions, parental health, school experiences and after school activities, and neighborhood characteristics. In each state and the District of Columbia, 2,000 households with children will be administered detailed interviews.

DATA LIMITATIONS: Each state's sample is 2,000 children which may limit multivariate analysis. Racial detail beyond White, Black, Multi, Other may not released at the state level to protect the identity of survey respondents.

STATUS: The survey will be conducted January through December 2003, and repeated in 2007.

HOW TO ACCESS DATA: The public use file will be released in mid 2004 on the NCHS website.
