Health and human services policies and programs have wide-ranging effects on individuals and communities which can facilitate optimal health and well-being or impose harm, particularly for historically underserved communities. Measuring progress toward achieving these goals can help staff understand the success of initiatives and inform future efforts across health and human services policies, practices, and programs. To help individuals measure the progress and success of equity initiatives, this tool describes (1) how to get started and (2) indicators of the progress and success of equity-focused initiatives across short-, medium-, and long-term outcomes.
