Use of electronic health information exchange (HIE) with and by long-term and post-acute care (LTPAC) providers lags behind that of physicians and hospitals. However, a few grant programs have supported HIE with LTPAC providers. In addition, Medicare payment changes (i.e., hospital readmission payment penalties and physician payment for transitions of care and chronic care management) are beginning to increase HIE with LTPAC providers.
