The Rosetta Stone spreadsheet was designed for the Department of Health and Human Services Office of the Assistant Secretary for Planning and Evaluation (ASPE) as a tool to assist long term care providers and clinicians in the transition to using Logical Object Identifiers Names and Codes (LOINC) and SNOMED CT as the terminology of choice for the documentation and conveyance of OASIS-C (based on CMS OASIS-C data specifications v2.00, Revision 3, October 2009) assessment content. The Rosetta Stone is a tool that can be used for various mapping scenarios:

  1. Map from OASIS-C in CMS format to corresponding Model of Use LOINC representation: In this scenario, one uses the Rosetta Stone "Model of Use" tab. For each OASIS-C question and answer, there is a corresponding LOINC question and answer code.

  2. Map from OASIS-C in CMS format to corresponding Model of Meaning representation: In this scenario, one uses the Rosetta Stone "Model of Meaning" tab. For each OASIS-C question and answer, there is a corresponding code from a standard terminology such as SNOMED CT or code set such as CVX. OASIS-C data items were also evaluated against other standard terminologies such as RxNorm. However, these other terminologies were not compatible with the data captured in this CMS standardized assessment instrument.

Tab 2 Model of Use Using the OASIS-C section and question ID as the primary sort order, this tab presents OASIS-C questions and answers (based on the CMS OASIS-C data specifications v2.00, Revision 3, October 2009) and corresponding LOINC representations and codes (provided by Indiana University of Regenstrief Institute, Inc.).

Note: Due to limitations of the OASIS data specifications, several “question” rows have been added to better represent OASIS question/answer pairs and to facilitate formatting in a CDA-based message. These added rows are identified by the use of italicized text for the description of the question.

  • Columns A - C: OASIS-C question and answer ID numbers and descriptions based on CMS OASIS-C data specifications v2.00, Revision 3, October 2009.

    Column A = CMS Question ID for each OASIS data item (This column is the primary sort order for the tab.)
    Column B = CMS Answer ID for each OASIS data item
    Column C = CMS Description for each OASIS data item

  • Column D: Contains some general guidance related to the data mapping.
  • Columns E - G: Contain LOINC representation of the OASIS data items.

    Column E = LOINC question code for the OASIS data item
    Column F = LOINC answer code for the OASIS data item
    Column G = LOINC answer set object identifier (OID) for the OASIS data item

Tab 3 Model of Meaning Using the OASIS-C section and question ID as the primary sort order, this tab presents OASIS Extract questions and answers (based on the CMS MDS2.0 data specification v1.01.0, December 2010) and corresponding:
  • SNOMED CT (version 20100731) representations and codes for semantically mappable MDS3.0 data items.
  • CVX (September 30, 2010 update) codes for MDS3.0 vaccination data items.

Notes:

  1. CAP SNOMED Terminology Solutions provided the SNOMED CT mapping of MDS3.0 data items. The American Health Information Management Association (AHIMA) provided the ICD-9-CM, ICD-10-CM, and CVX mapping of MDS3.0 data items.
  2.     Column G = SNOMED CT Fully Specified Name (FSN) for each assigned SNOMED code

  • Column H: Contains CDC Race and Ethnicity Codes for the OASIS race/ethnicity data items.
  • Column I: Contains Codes for Vaccines Administered (CVX) for the OASIS vaccination data items.
