  2. Although other mental health SDC programs might pre-date Florida SDC, documentation of these programs is scarce.

  3. The 1915b/c waiver combines a Medicaid 1915(b) Managed Care waiver with a Medicaid 1915(c) HCBS waiver.

  4. Real Choice Systems Change from the CMS support states' development of regulatory, administrative, program, and funding infrastructure for reducing individuals' reliance on institutional services and increasing their assess to HCBS.
