PROJECT DESCRIPTION:   The State will consolidate all current State and Federal funding streams for approximately 4,000 developmentally disabled adults under one managed care Title XIX waiver program. Eligible enrollees will be enrolled in a private HMO or approved health plan for acute health care. A clinical management system will assist enrollees in obtaining long-term supports. The State will assess long-term care services use and ascribe a dollar amount for procurement of these services.
